Upload instructs #44

Added section to readme.md covering basic details about the desired format of the monthly data and options for uploading the final files. I also updated the naming convention for the different methods to match the names used in the report.

I've included an embedded form on the gh-pages branch that will allow people without git lfs to manually upload files (http://ssj-delta-cu.github.io/ssj-overview/upload.html)

A couple of questions:

  1. Should the JPL-PT method be removed from the README? I don't think that it was included in the report.
  2. Likewise, should DETAW be added to the tables?
  3. Is there a way that we should request people to tag a commit to mark the output as a release version?
