Have you tried to enable "Routing fix" and/or "MSS Clamping" in app-settings?
(Please note: These options won't be visible if your phone/kernel does not
support these features.)

I have Enabled both "Routing fix" and "MSS Clamping". Neither seems to help
with the connection.
I have the above settings also. Device-Profile in settings has been changed to
Auto, also tried Generic as well.
Device is rooted.

Ok, I finally got it to connect here.
Still no internet access, but I have connected. Settings are as followed:
Device-Profile: Auto
Setup-Method: Auto
Enable WiFi-Encryption: OFF
Enter SSID: RootedAndroid
Select Channel: 1
Transmit Power: 3mW (5dBm) *I know it's low, but it always saw the connection*
Don't disable Bluetooth: OFF
Enable Access-Control: ON
Automatic shutdown: OFF
Change LAN: 192.168.1.0/24
MSS clamping: ON
Routing fix: ON
Change preferred DNS: 8.8.8.8 *default*
Change alternate DNS: 8.8.4.4 *default*
Client Notification Mode: All Clients *NOTE, I never got notified when I
thought I was connected*
Notification Ringtone: Freeze
Vibrate on connect: ON
Disable Update-Check: OFF
Disable Wake-Lock: ON
Battery Temperature: Farenheit
I hope this helps. I'll be glad to see the product working here also.

I have the same device and version of wifi-tether but I am unable to see the AP
at all. I've attempted connecting via netbook, laptop and tablet. It seems
that my device isn't even broadcasting, this is the stock rom perm rooted with
VIVID_CWM_ROOT_VER2.

Same here I have a rooted Vivid on AT&T I get a connected but no internet. Also
the phone actually shows only .04 down and 3. up and seems to stall.
This might sound a bit naive but why can't I just hack arround the internal
"you must call 611 to tether this device" warning and use the internal Wi-Fi
hot spot. I do not use enough data to throw any flags to AT&T.
