Lauren to confirm fields and request form from eDC:

Suggested fields: course and unit title, # students, unit code, # weeks and/or structure, duration, referencing style? Notes box? Needs to be able to include attachments: .doc/.pdf/.ppx/.txt documents

It might also be possible to drop the form into myCourse so lecturers could use from there as well as the Library website - RE to investigate once form available.
