Convert changelog to markdown #282

Description

The changelog was converted to markdown because it is more suitable for changelogs than asciidoc:

The link list can be written at the end of every release instead of the start.
A link contains the pull request number only twice instead of three times.
The changelog can be directly copied into the release page on GitHub which only supports markdown.
