Error on provisioning DEV: 'Agent did not show up ....'

Followed instructions to provision DEV for JKE sample app. All steps OK up until MySQL_Server and WebSphere_Liberty_Profile: These did not complete because the Agent did not show up even after 360.0 seconds.

Problem seems to be related to this (snipped from Instances output log): Starting Execute cloud user/final scripts...
