Improve guidance on Tax Documents page of Maker Portal #568

@hstove and @markmhx is it possible to have two buckets for collecting W8/W9 materials? Would be ideal to have one for U.S. based and one for international, if possible. This is a nice to have and not necessary for release.

Do you mean that we'd add a new tab to the Maker Portal (e.g. "Tax Materials") and then have the user indicate whether they are US-based or international (similar to https://github.com/blockstack/app.co/issues/565) before viewing a form for uploading tax documents?

What fields should the form have for both of these cases?

I'd suggest the new tab says "Tax Materials" then on the initial page it simply asks "Are you a US citizen or business?" with "Yes" and "No" as options that take the user to relevant pages with materials.

This looks generally good, though I'd tweak the guidance for a bit more clarity:

We need a completed IRS form before you can participate in App Mining.

Please submit IRS form W-9 if you are a US citizen or IRS form W-8 if you are not a US citizen.

The following buttons will take you to Dropbox where you can upload your form. We will review your upload manually and either indicate here that it has been submitted successfully or contact you if any corrections are needed.

and @GinaAbrams can you verify that we'd indeed contact them if corrections are needed?

I've also noticed that if I flag the company in the admin panel with "Tax information is collected", this "Tax Documents" UI doesn't change. We should remove the current UI and instead just show a message saying "Thanks! You've successfully submitted your tax documents" with the same visual treatment as the "Document Signing" confirmation state.

Both US citizens and non-US citizens must complete an IRS tax form before to particpate in App Mining.

If you are a US citizen, please complete IRS form W-9. If you are a citizen of another country, please complete or IRS form W-8.

Submit your completed form by uploading it to Dropbox using the button below. We will review your form and indicate your form is accepted here. If any corrections are needed, we will contact you directly.

@markmhx It is distracting to see that SEC link at the bottom --- what is it for? Is that intended as a copyright type thing required on each page? If so, another treatment may be more appropriate:

Let's make sure to please use Persons and not citizens.

"US persons" generally not used in this context -- is there a reason for this? Citizen is a specific designation I believe for tax purposes.

The SEC link at the bottom has been moved to a section on the left-hand side per work on https://github.com/blockstack/app.co/issues/567.

The latest staging for this screen now shows the following based on the state of dropdown selection:

My main concern with the copy remains that we don't indicate the "why" for this work (i.e. that it's required for App Mining participation). We may still want to add guidance on how we'll handle the Dropbox submissions, both when successful or not, depending on @GinaAbrams's input.
