Unify CAB minutes format and create template #103

This PR fixes #100 and updates the formatting across all meeting notes we have so far to match.

The template file was moved from the Economics CAB directory up to the main CAB directory, and some examples of the expected data were added as well.

The changes to each CAB document were cosmetic and no content was altered (outside of prettier preferring _ over * :shrug:).

The file names were updated as well to include the date and the SIP number being reviewed.

Tagging @obycode and @mattyTokenomics for visibility to the changes made in their CAB minutes, and open to any feedback.

It's exciting we're all converging on a standard for this next time!

Going to merge this in so the file names are set and we can link back to the minutes for SIP-015 review on the main branch.

If there is any feedback or changes anyone would like to see let's open a new issue/PR and hash it out there!
