Upgrade our Creative Commons license to CC BY 4.0 #201

We get occasional requests from people using our maps in academic journals where CC BY 4.0 is required (we are still on 3.0).

I don't see any reason we shouldn't just switch from 3.0 to 4.0 to make life easier on everybody.

Done in the interim update to the maps.stamen.com page that just happened. Note that the html file in this repo (https://github.com/stamen/maps.stamen.com/blob/master/index.html) no longer reflects what's on the web.
