EvidenceType description, data dictionary update, homepage update #106

I added a description to EvidenceType, added a section in the data dictionary and the homepage listing the metadata elements not included in the DD, updated the mCODE URLs in the DD since the canonical URL was changed, formatted the Cancer Coding picture so the text goes underneath the picture
