Request new LOINC codes for "disease status" and "cancer disease status" #113

Requested by Susan Matney re: CancerDiseaseStatus. The current LOINC code used is in "response to treatment" which differs from our CancerDiseaseStatus profile which is an assessment to the overall patient status and not specifically in response to treatment".
