Ignore warnings #118

Includes ignoreWarnings.txt file. Also changed the package to conform with new requirement for pattern like hl7.fhir.us.mcode (adding hl7). Also, updated a corrupted snomed cache file.

Visual review of code changes looks good. It's odd that you ended up w/ different caches than I did -- but as long as everything's working then I'm not too concerned.

I'll give it a run on my system and see how it goes.
