Form layout: Title

Add Order

Customer Name

As you type it shows a list of names in the DB
If entering a name that is not in the DB it will indicate under it that this name is not in the DB and will add it into the DB

Order Name | Div

both are text boxes, no validation required

Week Picker

When click on it displays a calendar where you can select the week of the month

Size

[None, Regular, Medium, Large]

Day1 - Day 5 (going vertical)

A or B radio buttons

Buttons

Add and Cancel
