Add issue templates #32

Issue templates help to help new users submit the information they need to while fixing bugs and feature requests. This makes the issue more readable and saves time asking the user to upload the required information beforehand.

@Antolius I didn't know about issue templates made in YAML. Thanks for the idea, I updated the templates to use YAML as they look more interactive than markdown.
