Add `location` parameter #683

[ ] Enable e.g. location='ghana' to set initial age distribution (& birth & death rates?)
[ ] Include warning if using defaults (location=None)
[ ] Pre-population to global averages if location=None?

One of the reasons this is difficult is that we'd have to store processed files for every possible location - we did do this for HPVsim but with a smaller subset of demographic files but even then it wasn't trivial
