Clarify getting started steps

Was trying to help someone try Statiq and this is some proposed changes based on the feedback from that experience.

• use --name instead of -o
• clarify add needs to be run within the project folder.
• clarify output folder location
• clarify preview source and http example
• remove an unused namespace
• add index markdown support
