Dynamic UI rendering of measurements #27

For #11. Added functions and a list of available measurements, and updated the single-child calculator to dynamically render UI elements based on the currently selected growth reference.

I'd like to merge this in now, so the functionality is available for @giordano current PR #26 (for #8). I will raise separate PR for adding dynamic UI to multiple-child calculator.
