For #12

Persist user identifier over browser sessions
Add uploading of sitar-style growth references in .csv format
Save growth reference files and persist for user
Hold growth reference dataframe as global variable
Add uuid package and update to R 3.6.2
TODO:
- add validation of uploaded file
- use new method of getting growth ref on centile and multple tabs

I've loaded this branch and checked it's up-to-date with origin, but the single test of get_height_sds fails! What am I doing wrong?

Sorry, test fixed! Remember to packrat::restore() to get the updates to packages.

Will merge this now so centile+multiple tabs can be updated in other branch. I'll work on validation in a new branch.
