Import initiative, add 3 new columns in excel file template #133

This PR fixes #89.

Added 3 new essential columns (exchange_rate, uses_only_own_funds, main_recipient) in Import Initiative excel file template. AE Marker - Project Import Template.xlsx

Added "DUMMY" value to continents, regions and countries. This is to fake the validation rules as they are required.

Continents, regions and countries will be empty for the imported initiative. User can edit the imported initiative and fill in them in front end.

We already have instructions to inform user to add additional details after import.

Live env deployment is completed. Performed testing with positive result.

Issue #89 is closed automatically after merging this PR.
