Add Institution-level roles #242

Currently, a user is assigned roles at the site-wide level. So an institutional admin is always an institutional admin. This is fine when a user is only linked to one institution, but there may be cases where a user should be institutional admin for Inst A, and an Institutional Assessor for Inst B.

This means we need to have roles at the site-level (for Site Admin and Site Manager), and the Institution-level.
