Phil had a number of comments on D1709. This PR addresses most of them, but leaves open some of the more fundamental issues that require further discussion by the group.
Also tried to adjust the formatting using standard LaTeX mechanisms. However, the library we have adopted to try to get standardese formatting essentially creates a complete ecosystem that replaces much / most of the functionality one would expect from the usual LaTeX packages. We probably need to decide whether to go all in with Niko's ecosystem or just borrow some of the formatting ideas. (I am leaning towards the latter, as I wasted a fair amount of time source diving to try to figure out how to do things like adjust spacing in section headings. And even when I found them they did not behave as I would have expect them to.)
