It is not necessarily a blockchain issue.. I have been trying to track down the cause. If you have any details to reproduce, please share here: https://steemit.com/steem/@timcliff/help-are-you-missing-anyone-in-your-follow-list
Thanks, Tim. I think I know only cases, where I have already re-followed the user. If I come across a user, I have not re-followed yet, I will share it in your post mentioned above in your comment. I see now, that eventhough the follow button is not showing I'm following the person, the person's post are showing in my feed and my account is listed in person's followers list.
I have just entered following comment on your above mentioned Steemit post:

I have opened issue on Github and here on Steemit in my post Follow Feature (Sometimes) Does Not Work - Bug?
I just realized one thing, which might help you, @timcliff, solving this problem. All the "problematic" accounts for me start with letter Z, meaning they are all at the end of the alphabet. I know some people who have trouble following me and I also start with letter Z.
It seems that if I follow many persons and try to follow newly somebody from the end of the alphabet, this doesn't properly show in follow/unfollow buttons. It looks like a time related issue in combination with number of people followed. Try to think in this direction, maybe there is something to it.
Example: I just started following @zainal.bacee for first time ever just for test. I see myself in his list of followers, but the follow/unfollow button says I'm not following him.
Based on your description it sounds like a condenser issue. If you have steps to consistently reproduce, I would suggest opening an issue in that repository.
Ok, I will.
I have posted an issue regarding following/unfollowing feature on Steem blockchain, which in some cases does not work fine. I start following some users, but eventhough everything looks fine, it turns out I am not following them.
