update TOCs of example notebooks

list of changes

• remove old HTML TOCs from notebooks (whereever present)
• generate new markdown tables using https://github.com/rusty-electron/jupyter-toc-generator. This is a fork of https://github.com/stopwhispering/jupyter-toc-generator with some changes. See details of commit here.
  • In order to achieve the same output, perform the following:
    • clone https://github.com/rusty-electron/jupyter-toc-generator
    • install using pip install . after cding into cloned dir
    • run generate_toc --offset 1 <path-to-notebook>
