main changes:

startup (and subsequent) is now much faster
no more walking the hashmap for any reason other than import
few bug fixes that were causing at best CPU spikes, at worst hangs
fixed offers, trust lines tables
created indexes where needed

note: this PR only contains the disabling of the node store backend cache
