Rails 4 #196

This is just a new PR containing all the hard work of @zeiv, @gordonbisnor and @aepstein.

It's basically a combination of #193 and #194, with a new nudge to get this merged back upstream :)

Thanks Steffen! I'm in a similar situation (trying to get things updated to Rails 4.1 for a new team on a project I'm finished with). Not prepared to say yes at this point, but in the short term hope to keep sending patches.​

For now, we can of course also handle it similar to this one: I will merge pull requests, which are tested and approved by one or two others
