Include type in facet name for disambiguation #575

At the present time, when one creates a Facet, no matter what type, the actual 
Facet in the left pane is without a unique name/identifier.

The attached image shows an example of two Facets made against the same 
column(named landline_home).  As you can see, there is no way to tell which is 
which.  In this case, one is a Duplicates Facet, and the other is a Blank 
Facet.  This is not so bad when you only have two of these Facets opened at any 
given time, but when you have several or dozens, it becomes difficult to 
remember which is which.

This need only apply to the Duplicates, Blank, etc. type Facets, where one 
cannot otherwise visually distinguish what type of Facet they are by simply 
looking at them.

a workaround is to click on change and close the transform windows without 
doing any modification. The grel expression is now displayed in the facet 
choice column. 

This apply to any facet.
