clarify that consumer field is required for free provider #19

Turns out spec was not very clear about the fact that "free" provider requires "consumer" field to be specified. I should also make a note that:

  1. Account MUST be granted provider if asked for the same "consumer".
  2. Account SHOULD be granted provider if previously granted "consumer/add" expired without been invoked.
