Create a contact form #1

I'm thinking of creating a contact us form that forwards any messages to core team. Derk, Kathy, Tim, and David.

I went ahead and created a "contact us" form that will automatically send emails to all 4 of us (original team) if anyone uses it.

If you do NOT want to get these emails, let me know and I'll take your name off the list.

I will leave this issue OPEN until I know that everyone is ok with this.
