Add middle name #20

This PR accomplished the following:

  1. Add middle name to user profile.
  2. Add middle name to checkout form.
  3. Updating user's profile with the display name for display of the middle name.
  4. Overriding PMPro's display of the user's display name.

This PR needs extensive testing in a cache-enabled environment and I'm sure I've missed some things, hence the draft PR.
