Has backward compatibility to keep existing functionality for 2.x, but has the following improvements for 3.0:

No longer needs custom cancel page template, can use hooks built into 3.0
Saves reasons to user meta instead of order notes
Adds panel to 3.0 edit member page to see all of a user's cancellation reasons listed in one place

Resolves #16
