MAINT: make Salmon pip install-able #127

What does this PR implement? It makes Salmon pip install-able.

[x] move /out/ to /salmon/out/ (as mentioned in https://github.com/stsievert/salmon/issues/126#issuecomment-996414048)
[x] define offline.txt
[x] implement two test environments for testing (one pip, one conda)
[x] document pip install
[x] upload Salmon to PyPI (automatically on release)
