Multi-doc support #9

This is ready. Please review and merge.

After a bit of testing we could potentially deploy this to notepad.substance.io. Then people can create and share their own pads.

Code looking good. I tried it out but don't think the colors are considered yet. Probably we are still missing a piece to make a user connected with one identity which is mapped to a color, right?
