"Add New" icon placement is off when scheduling a shipment

When scheduling a shipment the "plus" icon appears underneath the drop down
menu instead of next to the drop down menu, like it appears elsewhere. See
attached screenshot.

This happens at lower resolutions or when you resize the window below a certain 
size.

I would like to address this bug by removing the "plus" icons completely,  and
instead add an "Add New ..." option as the first choice in the select lists for 
these
fields.  This solution would also conveniently fix Issue 53.

For now we are resolving this bug by preventing wrapping.  In the future we may
implement the "Add New" as an option in the select lists.
