URLs in related items should always be hyperlinked from title #123

Currently relatedItem/titleInfo isn't hyperlinked using the link in relatedItem/location/url for certain types of related items. The link itself is also not displayed so this information is lost to the viewer. All types of related item should have the same behavior of linking the title.
