Display creator name as part of uniform title #41

LC uniform titles are name-title authorities -- without the name, they are not unique.

There are multiple uniform title authorities with that title and different creator names.

Desired behavior is to concatenate the values of the <name> and <title> elements with the same value in the nameTitleGroup attribute.
