Alarm always sounds at beginning of waking period

What steps will reproduce the problem?
1. Set alarm.
2. Place on bed.
3. Go to sleep.

What is the expected output? What do you see instead?
The alarm is expected to go off at a point during the "waking period" (the 
half-hour period before the set alarm time) when the sleeper is in an "almost 
waking" state. This occurred properly for the first few nights, but after that 
(a few weeks, now) the alarm always goes off at the very beginning of the 
waking period. If the alarm is set for 7:00, it will go off at 6:31. If it is 
set for 8:30, it will go off at 8:01. This occurs regardless of the state of 
the phone (display on/off), position on the bed (head, foot, under pillow, 
upside-down), or state of the sleeper (near-waking or in deep sleep).

Additionally, the history bars always appear to be solid blue, with no 
variation throughout the night. It's unknown whether these two issues are 
related.

Which phone model do you have?
Motorola Droid, Android v2.2 build FRG22D

Please provide any additional information below.
The phone is always charging (plugged in to power outlet) during the night.

I also see both issues (alarm always at beginning of wake period and solid blue
