What steps will reproduce the problem?
1. Create a new account
2. Immediately edit the account
3. Add a value to a field
4. Save the record
5. Go back to the accounts list
What is the expected output? What do you see instead?
The expected output would be to see the account that was edited updated.
Instead a second account with the same name is created.
