Some compendium items may not be properly migrated, but artifacts of the failed migration are in the system

I noticed today that one of our homebrew cults, which was in a compendium, was lacking correct pointers to its spells and spell descriptions. Thinking I may had accidentally done something wrong, I set about recreating the rune spells and descriptions.

In doing so, when creating the RQID to link the spell description to the spell item, the RQID screen presented a warning that the spell already existed, with a path the compendium stating where the other RQID was.

However, inspecting the compendium visually does not show the item that the RQID system thinks is there.

How: To Reproduce

I am not sure how to reproduce this as it "was working fine" before we recently migrated to V12 and updated the RQ system to afterwards.

Expected behavior

The RQID system should not say there is a duplicate item when there isn't.
