TODO: remove whefs_inode::name member #4

The whefs_inode::name member causes us all sorts of minor grief, and should
probably go away. The main reason i don't strip it out this moment is
because doing so would break whefs_file_name_get().

whefs_inode::name was removed in r168. i had to add whefs_file::name as a 
temporary
workaround, but that will also be removed at some point.
