The following FM features must be implemented:

[X] Key on and off
[X] Pitch calculation and write
[X] Volume calculation and write
[X] Channel LFO and panning flags
[X] Global LFO parameters (can be set with the fmreg command)
[x] FM3 special mode (#13 )
[x] DAC enable/disable (#4 )
