regarding default template:
- copied updated default-2019/form-paypalcheckout.htm to default/form-paypalcheckout-div.htm
- copied changes from default-2019/form-cartprogress.htm to default/form-cartprogress.htm (don't think it's in use)
- copied changes from default-2019/form-cartpostsale.htm to default/form-cartpostsale.htm (good in QA)
updated html-header.htm with new master css file
please fix the thank you page alignment (see below)
