Add :model_name option which allows custom override of model name #15

Also includes some gemspec maintenance and code cleanup. (The last commit is the :model_name feature)

I believe this is a better/more flexible implementation than PR #12, as it can handle both subclasses and namespaces.

Looks good! Lets merge it when the :fish:es are fixed :)
