Fixed issue introduced in 1.4.7 where customer could not place order if standard Magento shipping was used and the customer did not update anything in checkout before placing order.
Fixed issue where URL to action 'ReloadShippingMethods' could be incorrectly generated.
Removed an unused JS file.
