An attempt at solving https://github.com/sveltejs/svelte.technology/issues/359

It's a bit hacky with the side effects within replace and such, but perhaps you'll allow it or let me know if some ideas for tidying. Thanks

Hmm, I later thought perhaps this could be done with CSS pseudo content elements. I'll have a go at that if I have some time.
