Various macOS fixes and improvements #99

The changes are as follows:

Fix an issue with a define preventing Doom64EX from building on macOS
Fix an issue when attempting to do a 'make install' - it now looks in the right location when installing the binary
Created an ICNS file and added it to the macOS app bundle (it will now show the Doom64EX icon instead of the default blank icon)

I haven't got any real experience with C++ so not sure whether there's a better way of doing what I done so any suggestions appreciated.
