The Name, Email, and Affiliation boxes could be made longer.
The response box for the question "How did you hear about this course?" could be a resize-able box like the "Tell us about yourself" box below it. That way registrants can put a bit more information in there.
After clicking the Submit button, one is redirected to a Google Forms thank you page. If there was some way to avoid that redirect and go back to the SW training page, that would be helpful. I was a bit confused when the redirect happened, partly because I had not realized the page is actually a Google Form.
