Create whitelist facility

As the fediverse gets older and wiser, potential admins will learn that starting an instance is potentially a huge undertaking.

Here's two features people have been asking for for a long time that would make it possible for would-be admins to more easily keep their instances safe(r).

• defederation: the ability to make an instance that has no connections to other instances at all
• whitelist-only instances: federation only to whitelisted instances that the admin considers safe

A server with whitelist enabled and an empty list of whitelisted servers would be defederated. This seems a better approach than implementing the two features separately
