Release 2.0.1 #106

This includes a number of minor fixes -- please see the commit log. All tests pass, and most of the code has been running successfully for 6 months or more.

I've added a CHANGELOG.txt that gives a more readable version of the changes. Since the code has mostly been running well for months and all tests pass, I will now merge it to master and publish as 2.0.1.
