Guardian's heart does not give you the shield until you reach the next stage sometimes.

This sounds like a bug, likely caused by another mod - I'll experiment to see if I can replicate, but a quick glance over the code isn't offering any suggestions as to what could cause this. Let me know if you manage to replicate.

Oh. I do wonder what would cause it since I dont have too many item mods that could mess with it.

I'm not sure either, but it runs a recalculatestats when picked up, and the additional shield is also handled via the same API. ¯_(ツ)_/¯
