Not sure what causes it exactly. IIRC it isn't just visual.

Seems to happen because of the "toxin not ready" debuff not being on a character while the "toxin ready" buff is also not on a character. Had it happen a few times, my guess is it's because of playing as a multiplayer client w/ ping.
