Re-implement completion for role targets #823

This re-implements role target completion for reStructuredText and for the first time in MyST syntax. This isn't as feature complete as 0.x yet, (no intersphinx, :ref:`alias <label>` not handled yet) but the overall architecture should be there ready to build on
